How Do I Apply?

To sign up for a Health Savings Account is as easy as 1, 2, 3!

1. Determine if you qualify for a federal tax-deferred HSA.

2. Print and sign HSA application form

3. Send the application form and a check (payable to American Health Value, LLC) for the Annual Fee and your Initial Deposit ($10 minimum) to:

Boise, ID 83707-2063

Within ten to fifteen business days, your AHV VISA Debit Card will arrive in the mail.  You will also receive 50 free personalized checks for your HSA account.

 
Health Savings Account Rollover Procedure.

For those of you who already own an HSA, we make it easy to roll your account over to American Health Value.

1. Print the Account Rollover Form and Checklist. *

2. Mail to American Health Value, LLC,
PO Box 8063, Boise, ID 83707-2063.

* Federally Qualified HSA: To have a Federally Qualified HSA, you must purchase and maintain a high-deductible insurance policy and you cannot be covered by another low-deductible insurance policy.
