1. Determine if you qualify for a federal tax-deferred HSA.



2. Print and sign HSA application form.



3. Send the application form and a check (payable to American Health Value, LLC) for the Annual Fee ($36) and your Initial Deposit ($10 minimum) to American Health Value at the following address:

Within ten to fifteen business days, your AHV VISA Debit Card will arrive in the mail.  You will also receive 25 free personalized checks for your HSA account.
