With all of these locations, finding a vision provider is really easy to do!

How To Use Your Card:
Using your vision discount card is easy, and you can either do it over the phone or online!
Locate a Vision Provider by Phone: Contact the customer service and support team at 800-342-7188. Tell them you are using a "WellCard Discount Card with Outlook Vision", and they will walk you through the process.
Locate a Vision Provider Online: Click on the button below to locate a vision provider using your zip code. Once you select a provider, call them directly to schedule your appointment. Identify to the provider, you are an OUTLOOK member using the WellCard Discount card. You will be able to confirm pricing for your procedures at this time.
No matter how you choose to set up your appointment, once it's scheduled all you need to do is show up, present your discount card and recieve your discounted rates at the point of purchase.
